Catalyst for Improving the health and wellbeing of local residents

In 2008, the Healthier San Joaquin County Community Assessment began including in the report several stories of local agencies in San Joaquin County that are making a difference to improve access to health care for local residents. Collectively called How We’re Making a Difference, these exciting accounts of local success have been assembled and are featured in this document. The following stories show how community initiatives and agencies are making a difference in San Joaquin County. 

Priority Goals

Utilize a process that will engage local stakeholders

Generate knowledge and findings that could lead to collaborative project development

Develop a comprehensive community dissemination plan

Assess both community needs and assets

Identify information and data that would be useful for policy and advocacy work

Provide a mechanism for ongoing tracking and monitoring
